Instructions for Completing the FSSAI Enquiry Form

The candidate has to enter his/her full name, as required in the form. The full name shall then be typed without errors and without any extra characters. The applicants are also required to provide their full names in order to facilitate identification of the applicants.

Please ensure that your mobile number does not consist of any country code, such as ‘+91,’ or any special characters. Furthermore, it should be an operational mobile number through which any possible clarification can be obtained. Furthermore, any incorrect number can lead to failed attempts. Also, any wrong number can cause unsuccessful attempts.

A valid contact email address should be provided by the applicant. The response or follow-up communication regarding the inquiry, acknowledgement, or follow-up communication, whichever, can be made to this email ID. It is the responsibility of the applicants to ensure that this contact information is provided and remains accessible.

Enquirers are required to state clearly the nature of their enquiry. This must be attended to by including every important aspect that might be applicable in this matter regarding reference or previous correspondence, if applicable. Inquiries on the same matter are supposed to be avoided by submittals.
